Situation

Mizell Memorial Hospital was first established in 1949 as a small rural hospital operating with a 29-bed capacity. To meet the needs of the community they needed more beds and more services. These changes in services included expansion of the facility with new additions and additional beds. Added services included nuclear medical services, a sleep center, senior behavior health care, a wellness center, and a clinic for primary care.

With no professional resources in the local community with knowledge to code and complete patient charts, help collect outstanding balances, and other daily RCM functions they partnered with Allegiance Group. The Allegiance Group partnered with Coronis (previously Avec) to provide professional coders to assist with the coding demands of the facility. With the continued success of our coding team, Allegiance expanded our services by adding an RCM and full lift outsource of the business office. This includes cash posting and day one billing.

Solution

With the assistance of Coronis coders and the RCM team, Mizell now has all major insurance payers set up to send and receive electronic claims and payments which were previously sent by paper. Coronis has also helped to streamline the electronic claim submissions by utilizing a claims scrubber (Waystar) and improve the clean claim rate.

The average maintained weekly clean claim rate is 90% and above. Coronis RCM staff has also assisted with chargemaster updates and the complete transition of Mizell Memorial clinics to Rural Health Certified. This included new payer codes, chargemaster setup, and NPI registration in the host system as well as the claim scrubber. Coordinated efforts between Mizell staff and Coronis staff to update existing accounts with the new RHC information.

This included the coding team and RCM teams to re-code accounts to RHC standards and re-bill all accounts with the new RHC information which totaled more than 500 accounts and $100K. Most recently Coronis staff coordinated with Mizell staff to improve operational effectiveness when billing for therapy and recurring accounts. The process was simplified and in return has made the process from admission to billing more efficient for all parties involved.

Continued Success

We at Coronis continue to help streamline RCM operations and processes with the Mizell business office staff. When the Coronis RCM team was added to Mizell they were adjusting 100% of all accounts over 365+ days in age, not pursuing denials. Coronis now collects every possible dollar on aged accounts. Our goal is to assist and improve our client’s collections on outstanding balances and reduce their AR aging by improving operational effectiveness.

We have added additional staff to help with our collection efforts and reduce the overall aging of patient accounts and continue our partnership with Mizell to improve all processes. We are excited and up to the challenge to partner with facilities that want to grow and be successful!

Results

Beginning in April of 2021, our team began a robust project to rebill 6 months’ worth of Medicare claims. Our team developed a rebill workflow and plan to deploy 8 additional FTE’s to work specifically on the rebill project. In just 90 days, our team was able to rebill all of the 7,000+ Medicare claims and reached an all-time high for GRMC collections of $5.7 Million.

We have successfully turned the tide of the damage from the CAH transition and months of shortages and have things back on track. We could not have done it without attentive offshore resources, communication between all parties at Coronis, and maintaining a good rapport with the remaining staff at GRMC.

The Coronis coding team also partnered with GRMC, where we successfully initiated ER coding and successfully intercepted all specialty coding namely (OBS, EMS, Surgery, IP-DRG, Cancer Center, and Ancillary coding). Coronis coders assisted the AR team in terms of coding and billing denials and assisting cashflow as well as managed to cover backlog volumes by working over the weekend and helped to reduce the DNFB.

The Coronis coders consistently maintained a coding accuracy of >95% within HFMA guidelines.

Our Ambitious Goal

We started the rebill project with a 90- day timeline, based on the 8 representatives working 35 accounts per day. Our team exceeded that expectation and rebilled all of the claims in 2 months’ time. We averaged close to 55 accounts per day which is a stellar production number. In addition, we also promised an AR Surge to help GRMC get extra money in the door before the end of their fiscal year in June 2021. We were prepared to deploy additional FTE’s and develop a work plan when the facility was overwhelmed and needed help. Because we had a good, cohesive working relationship with the client, we were able to deploy this quickly. They trusted that we would deliver and complete this great undertaking of rebilling over 7,000 claims. The RCM leadership team developed a workflow/handoff diagram reflecting the process of how work is coordinated between the Coronis team and the GRMC team. What made us successful in mitigating the inventory is compiling a full roster of the FTE’s we have assigned to the account. The workflow reflected the owners of each process in our scope of work while outlining the steps involved in the communication process between our team and the GRMC team.
